Whether your goals involve weight loss, weight gain, sculpting and toning, training during pregnancy, or adopting a healthier lifestyle, I am dedicated to helping you achieve them. I acknowledge that no credit nor refund shall be given due to a missed session on my behalf aside from instances referenced by the Personal Training Policies.

II.
I acknowledge that a 2-hour notice is required to cancel or reschedule a session without penalty. Any sessions canceled by the trainer will be rescheduled without penalty to complete package fulfillment. Each personal training session or plan is to be paid for in full before the session(s) begin. A training session series can be ended on my behalf when give 14 days' notice to my trainer.

III.
I, understand and accept that if I am 15 minutes late or more, per session that you are late, there will be a $20 late fee that will be needed to be paid in full before your next scheduled workout session.

IV.
I, understand and accept that if I reschedule a workout session in less than 24 hours notice, per session that you reschedule, there will be a $30 fee that will need to be paid in full before your next scheduled workout session.

V.
I, agree to be trained by Essence Yates, an ISSA-certified personal trainer, and decline the right to engage in penalty or lawsuit for any injuries that occur.

VI. In consideration of participating in the personal training sessions offered by Essence Yates, I, hereby release, indemnify, and hold harmless Essence Yates and any affiliated personnel from any claims, liabilities, damages, or injuries that may arise during or as a result of the training sessions.

VII. I, affirm that I am in good health and have not withheld any information regarding pre-existing medical conditions from Essence Yates. I further acknowledge the importance of consulting with a qualified healthcare professional before engaging in any physical activity and certify that I have obtained such clearance.

VIII. I understand that Essence Yates is not a licensed healthcare provider, and any advice or guidance provided during personal training sessions is not a substitute for professional medical advice, diagnosis, or treatment. I agree to seek medical advice from a qualified healthcare professional for any health-related concerns.

IX. I, agree to adhere to all safety guidelines, instructions, and recommendations provided by Essence Yates during training sessions. Failure to do so may result in termination of the personal training services without a refund.

X. Essence Yates reserves the right to modify or update these policies, and any such changes will be communicated to clients promptly. Continued participation in personal training sessions implies acceptance of the updated policies.

XI. This agreement constitutes the entire understanding between the parties and supersedes any prior verbal or written agreements. Any modifications to this agreement must be in writing and signed by both parties.

By signing below, I acknowledge that I have read, understood, and agree to abide by the terms and conditions outlined in this agreement.
